Help Us To Help You

Please let us know if you change your name, address or telephone number. Please do everything you can to keep appointments. Tell us as soon as possible if you cannot. Otherwise, other patients may have to wait longer.

We need help too. Please ask for home visits by the doctor only when the person is too ill to visit the surgery. Please keep your phone call brief and avoid calling during peak morning time for non-urgent matters. We ask that you treat the doctors and practice staff with courtesy and respect.

Please carefully read our practice booklet. This will help you to get the best out of the services we offer. It is important that you understand the information given to you. Please ask us questions if you are unsure of anything. Remember, you are responsible for your own health and the health of your children. We will give you our professional help and advice. Please act upon it.

Comments/Complaints

Our aim is to provide the best possible quality of care within the limits of the financial constraints imposed by the NHS. Our intention is to carry this out in a pleasing and courteous manner. We conduct patient surveys to assess the quality of hospital services and practice performance.

If you have any complaints or comments please contact the practice manager, Karen Clarke, who will be happy to see you, although an appointment may be required. We have an in-house complaints procedure.

Patient Participation Group

We have set up a Patients Participation Group. Members of the group are generally patients at the practice and also represent a variety of local community groups. If you have any comments you would like the group to discuss or if you are interested in becoming a member of the group, please contact the practice manager, or arrangements can be made for a member of the group to contact you. Information about PPG meetings is posted on the notice board to the left of the reception desk, along with copies of the previous minutes.

Patient Confidentiality And Data Protection

We ask you for personal information so that you can receive appropriate care and treatment. This information is recorded on computer and we are registered under the Data Protection Act. The practice will ensure that patient confidentiality is maintained at all times by all members of the practice team. However, for the effective functioning of a multi-disciplinary team it is sometimes necessary that medical information about you is shared between members of the practice team.

Freedom Of Information - Publication Scheme

The Freedom of Information Act 2000 obliges the practice to produce a Publication Scheme. A Publication Scheme is a guide to the "classes" of information the practice intends to routinely make available.
The scheme is available from the practice manager. Please request a copy in writing.

Zero Tolerance

This practice operates a zero tolerance policy. Any person found to be violent or abusive to any of the doctors, staff, visitors or another patient on the practice premises, will be removed from the practice list immediately and placed on a register for violent and/or aggressive patients.
