Register With The Practice?
To register as a patient at this
practice you simply need to bring in your medical card. This will have been sent
to you when you registered at your old practice. If you cannot find this card,
don't worry too much as we have forms you can fill in instead. Providing you
meet the relevant criteria, ie you live within the practice boundary, that there
has never been any breakdown of relationships previously, etc, there should be
no reason why we will not accept your registration.
Patients are now no longer registered with an individual doctor but with the
practice.
However, please be advised that even though we will make every effort to give
you an appointment with your usual doctor, this may not always be possible and
there will be times when you are offered an appointment with another doctor.
If you wish you may also request to see a different doctor to your usual doctor.
You simply need to express this wish to the receptionist when booking your appointment
and every effort will be made to comply with your request.
